All candidates are required to fill out the same form so that comparisons can be more easily made. MS Word Format1. Complete the form at your own leisure using the Tab key to progress between fields. All instructions are included in the form. 4. Adobe Pdf Format1. If you previously began working on your MIP through the .pdf link that was on this website, click the link to complete the form you started online. The Adobe format saves the document to our server; you may access it at any time with the password you provide. Please be aware that a default setting in the Adobe program limits the number of words that can be visible when printing your submission. Make sure your whole answer is visible on your screen.
